this is how it works

Lex Sarah is an important part of our quality work and a safeguard for both clients and staff. It obliges us, as a personal assistance provider, to report, investigate, and address any misconduct or risks of misconduct within our operations. The purpose is to prevent, detect, and correct shortcomings, ensuring that all services under the LSS (Act concerning Support and Service for Persons with Certain Functional Impairments) are carried out with high quality and in accordance with legal requirements.

What is misconduct?

Misconduct can be something like:

  • Exposes a client to the risk of harm
  • Contradicts applicable laws, regulations, or procedures
  • Involves shortcomings in care, security, safety, or treatment
  • In any other way threatens the quality of the service provided

Misconduct can occur through actions or through failures to act. It may involve anything from shortcomings in daily care to physical, psychological, or financial abuse

Who is responsible for reporting?

Everyone working within our organization, regardless of type of employment or assignment, is obligated to report suspected misconduct or risks. Reports must be made immediately to the responsible manager or directly to the operations manager.

What is the process?

1. Reporting

Anyone who discovers or suspects misconduct must report it, either in writing or verbally, to the responsible manager or head of operations."

2. Immediate actions

If necessary, immediate measures are taken to ensure the client’s safety and security.

3. Investigation

We investigate the incident in accordance with the regulations of the National Board of Health and Welfare. The investigation is carefully documented and covers the course of events, causes, and risks.

4. Action plan

Following the investigation, a plan is established to remedy the deficiency and to prevent its recurrence.

5. Notification to IVO

Should the investigation indicate a serious misconduct or significant risk, a notification is made to the Swedish Health and Social Care Inspectorate (IVO) in compliance with Lex Sarah.

6. Follow-up

We monitor to ensure that the corrective measures are effective and that similar incidents are prevented from recurring.
